A Beginner's Guide to Trying to Conceive

Understanding the mechanisms of fertility treatment is the first step! We'll introduce the basic knowledge of fertility treatment in an easy-to-understand way.

Did you know?
How to increase your chances of getting pregnant

To increase your chances of getting pregnant, it's essential to accurately determine your ovulation day and time intercourse accordingly. Many people still have the misconception that you can determine your ovulation day simply by measuring your basal body temperature, but in reality, ovulation has already finished by the time your basal body temperature rises.
Ovulation prediction test kits can predict the day of ovulation, when the chances of conception are highest, approximately one day before ovulation. It is recommended to predict the day when you are most likely to conceive and have intercourse at that time.

The relationship between basal body temperature and ovulation day

If you're starting to try to conceive, the first thing you should do is record your basal body temperature.
"Basal body temperature" refers to the temperature you measure after waking up, while remaining still. You measure it at a consistent time each day (between 5 and 8 am) by placing a thermometer under your tongue while lying down. Since precise data is needed, it's best to use a basal body thermometer that measures temperature in 0.00 degree increments.
Healthy adult women experience two phases of basal body temperature due to hormonal balance: a high-temperature phase and a low-temperature phase. Ovulation is often thought to occur on the last day of the low-temperature phase, but it can usually only be confirmed after the transition to the high-temperature phase. Therefore, it is said to be difficult to accurately predict the ovulation day based solely on basal body temperature.
However, knowing your body's condition through basal body temperature is very important when trying to conceive. It allows you to understand which stage of your cycle you are in, and it also helps regulate your lifestyle, which in turn contributes to managing your physical condition to make you more likely to get pregnant.

Next, I will explain why basal body temperature is divided into a "low-temperature phase" and a "high-temperature phase."
When menstruation begins and the follicular phase starts, basal body temperature drops to a low-temperature phase. Estrogen (follicle-stimulating hormone) secretion also increases. During this time, follicles mature due to follicle-stimulating hormone.
After that, the body goes through the ovulation phase, followed by the luteal phase. During the luteal phase, progesterone is secreted to make it easier for a fertilized egg to implant in the uterine lining. This raises body temperature, so the basal body temperature is in the high-temperature phase. If pregnancy does not occur, the amount of progesterone secreted decreases, causing body temperature to drop and menstruation to occur.
Because of this process, if you plot a month's worth of basal body temperature on a line graph, it usually divides into two phases: the "low temperature phase" from the start of menstruation until ovulation, and the "high temperature phase" from ovulation onward.

your body
Knowing this is key.

If you currently have an irregular menstrual cycle or severe menstrual cramps, you should be careful. For example, a menstrual cycle of 24 days or less may indicate anovulatory cycle, while a cycle of 39 days or longer may indicate amenorrhea (absence of menstruation). Also, severe abdominal pain during menstruation may indicate endometriosis, and blood clots during menstruation may indicate uterine fibroids. Cervical cancer is also increasing among women in their 20s. Try recording your basal body temperature to visualize your physical condition and rhythm.

If your blood pressure is following a curve like the one shown below, you should consult a doctor.

The high-temperature phase is short, lasting less than 9 days.

Even if you have distinct low-temperature and high-temperature phases, if the high-temperature phase is short (less than 9 days), it may indicate "luteal phase defect," where progesterone secretion is not functioning smoothly. Progesterone secretion, which raises basal body temperature, is essential for conception. You should consult a clinic.

High temperature phase lasts for 16 days or more

If your high-temperature phase, which normally lasts about 14 days, continues for 16 days or more, it could indicate pregnancy. Furthermore, if bleeding occurs after the high-temperature phase continues for 21 days or more, it could be a threatened miscarriage. If your high-temperature phase persists, it's best to get checked out at a clinic for peace of mind.

High temperature phase cannot be confirmed.

If your menstrual cycle is either too long or too short, and your temperature chart is mostly flat, it could indicate anovulatory menstruation, where menstruation occurs but ovulation does not. However, this can sometimes be temporary due to stress or other factors, so continue measuring your temperature for two to three months. If you still cannot confirm a high-temperature phase, it's a good idea to see a doctor.

Knowing your ovulation day can increase your chances of getting pregnant.

The "most fertile time" for conception is the five days from three days before ovulation to the day after ovulation. Of these, the one to two days before ovulation is considered the most fertile time for sex. Ovulation usually occurs about once a month (each menstrual cycle). Furthermore, the egg released during ovulation can only survive for about 24 hours. On top of that, sperm only live for about three days after ejaculation. Therefore, the five days from three days before ovulation to the day after ovulation can be said to be the "most fertile period."
If you want to have a baby soon, it's important to know your ovulation day in advance, as this is when your chances of getting pregnant are highest.
Ovulation prediction test kits can predict ovulation about one day in advance, allowing you to know the 1-2 days before ovulation, which is considered the most fertile time for conception.

Basal body temperature is only an indicator of ovulation.

Ovulation is often said to occur on the last day of the "low-temperature phase," but ovulation can usually only be detected after the body transitions to the "high-temperature phase." Considering this, it is difficult to predict ovulation based solely on basal body temperature.

However, it is certainly true that there is a close relationship between basal body temperature and ovulation day.

The "ovulation period" is the "low-temperature phase" of basal body temperature, but some people have a "lowest temperature day" where their temperature drops sharply. The probability that this "lowest temperature day" is the ovulation day is about 50-60%, and in reality, it is said that "ovulation occurs within a few days of the lowest temperature day."
Also, some people experience ovulation pain or bleeding around the time of ovulation. Knowing your own symptoms will make it easier to pinpoint your ovulation date.

After tracking your basal body temperature for two to three cycles, you'll gradually begin to understand your own rhythm, noticing things like, "Is it time for my high-temperature phase now?" or "It's almost time for my lowest temperature!" It can also be useful as a barometer for predicting the next cycle and understanding your physical condition. Above all, it might be a good idea to use basal body temperature as a "guide to determine your ovulation day."

The most fertile time for conception is said to be 1-2 days before ovulation, and you can predict your ovulation day in advance using an "ovulation prediction test kit." The test works by applying urine to the test strip and examining the change in LH concentration in the urine to predict ovulation in advance. LH is normally secreted in small amounts, but it is secreted rapidly before ovulation occurs, and this characteristic is utilized. You can easily purchase these tests at drugstores with pharmacists on staff, so feel free to use them in conjunction with tracking your basal body temperature.
If you're having trouble conceiving despite timing intercourse correctly, it's important to consult a clinic or other medical professional.

What men can do

The most important point for men trying to conceive is to "avoid accumulating too much sperm and not abstain from sex."
The testes produce new sperm every day, but if you let them accumulate for too long, the quality and motility of the sperm will deteriorate. If you are hoping to get pregnant, it is important not to abstain for more than 5 days and to ejaculate at least once a week to release sperm. In preparation for your fertile period, make it a habit to produce as many fresh and active sperm as possible.

Let's try the "Timing Method"

What is the timing method?

Having sex during the ovulation period, when conception is most likely, is called the "timing method." It is said that repeating the "timing method" every month can increase the chances of getting pregnant.
An egg can only be fertilized within 24 hours of ovulation. Therefore, the purpose of the "timing method" is to predict the day of ovulation and ensure that sperm meet within 24 hours. Furthermore, since sperm also have a lifespan, the timing of when sperm and egg meet is crucial.
The most fertile time for conception is from one day before ovulation to the day of ovulation. Since sperm are produced every day, it's a good idea to have intercourse several times leading up to ovulation. Aim to increase your chances of conception by timing intercourse as accurately as possible.

The process of timing yourself

1. Keep a basal body temperature chart.

Tracking your basal body temperature is fundamental to trying to conceive. You may also need a basal body temperature chart when visiting a clinic. If you are trying to conceive, be sure to keep a record.

2. Pay attention to the changes in your body.

Before ovulation, some women experience increased vaginal discharge, breast tenderness, or ovulation pain. Pay attention to these small changes in your body.

3. Predict your ovulation day using ovulation prediction test kits, etc.

Ovulation prediction test kits can tell you your ovulation day about one day in advance.

4. Have sex before ovulation.

Have sex based on your ovulation date, which is determined by your basal body temperature or ovulation prediction test kit. The most fertile time is from one day before ovulation to the day of ovulation. If you have been trying to conceive for more than six months without success, or if you are over 35, it is recommended that you visit a clinic as soon as possible.
